Claim reporting process, Clients can advise Babereki Insurance Brokers of a potential claim in the following ways;

  • Email
  • Telephone
  • Fax

Funeral Claims (BOPEU/MOONLIGHT/BKL)

1. BOPEU & Babereki Ka Lorato

  • Certified Death Certificate
  • Birth Certificate for minor (Under 16 years)
  • Certified ID Of Main Member or Claimant
  • Certified Copy Of Marriage
  • Certificate Affidavit On Different Surnames – Case By Case
  • Latest Bank Statement, BLIL Reward Card – For Benefit Pay Out
  • Claim Payable With 48 Hours Of Submission Of All Required Documents
  • The Claim Should Be Lodged Within 6 Months.
  • Police Report for Accidental Deaths

2. Babereki Moonlight Funeral Repatriation

  • Certified Death Certificate
  • Birth Certificate for minor (Under 16 years) or proof of age
  • Certified Passport Of Main Member or Claimant
  • Certified Copy Of Marriage / Affidavit signed by Local Chief  (home Country)
  • Latest Bank Statement – For Benefit Pay Out
  • Claim Payable With 48 Hours Of Submission Of All Required Documents
  • Police Report for Accidental Deaths

The claim should be lodged immediately after death

All Risk Claim

  • Completed Claim Form
  • Two Repair/Replacement Quotations
  • Police Report and/or statement were applicable

House Owners Claims

  • Completed Claim Form
  • Police Report were applicable or report from relevant authorities depending on loss
  • Two Repair Quotations
  • Please note additional documents may be requested if applicable

Motor Accident Claims

  • Completed Claim Form
  • Copy Of Registration Book
  • Copy Of Driver’s License (Driver at time of accident)
  • Two quotations for repairs
  • Police Report and/or statement
  • Please note additional documentation may be requested if applicable

Windscreen Claims

  • Completed Claim Form
  • Two Repair Quotations
  • Copy of driver's license (Driver at time of accident)
  • Police Report were applicable

Public Liability Claims

  • A Brief Write-Up Outlining What The Claim Is About, Including The Date The Incident Happened And A Brief Description Leading To The Loss.
